How do I obtain my medical records?
To request a copy of medical records, please complete the Authorization Form. Email the completed form to medicalrecords@thelittleclinic.com or fax it to: 615-425-4344.

Insurance
Does The Little Clinic accept my health insurance plan?
The Little Clinic accepts most insurance plans and networks. Please contact your insurance carrier prior to your visit to verify coverage, co-pay responsibility and that your insurance plan is participating with The Little Clinic. Confirming this information in advance will help avoid any unforeseen charges.
What if I don't have health insurance or have a plan that The Little Clinic doesn't currently accept?
No problem. You have the option to pay for your visit by debit card, major credit card, cash, money order or traveler’s checks. We also accept Near Field Communication (NFC) payments, Apple Pay, Google Pay and Samsung Pay inside our clinics.
If The Little Clinic files a claim with my health insurance plan, how does it work?
The clinic staff will collect the necessary co-payment from you as specified on your health insurance card. The clinic staff will make a copy of your photo identification and health insurance card for medical records and billing purposes. A claim will be submitted to your health insurance company on your behalf. After the insurance company makes a payment to The Little Clinic, we will send you a bill for any amount deemed your responsibility by your health insurance plan (verified in your Explanation of Benefits provided by your health insurance company).

When is The Little Clinic open?
The Little Clinic has convenient extended hours seven days a week to fit your busy schedule. The clinic hours are:
Monday-Friday: 8:30 am – 7:30 pm
lunch 1:30 pm - 2:15 pm
Saturday: 8:30 am – 5:00 pm
lunch 1:30 pm – 2:00 pm
Sunday: 9:30 am – 4:00 pm
lunch 12:30 pm – 1:00 pm
Payments
What form of payment does The Little Clinic accept?
The Little Clinic accepts debit card, MasterCard, Visa, American Express, Discover Card, Apple Pay, Google Pay, Samsung Pay, cash, money order, personal check or traveler’s checks.
Can I pay my bill online?
For your convenience The Little Clinic now offers payment access online. In order to pay your bill online you will need The Little Clinic account number and your bill amount listed on your copy of the bill you received. If you cannot locate your account number or bill amount please contact customer service toll-free at 888–852–2567 We currently accept all debit/credit cards for: Visa, MasterCard, Discover, and American Express. Our board-certified nurse practitioners and physician assistants provide high quality, affordable healthcare conveniently located inside your neighborhood grocery store. The Little Clinic treats patients 12 months and up.
- The Little Clinic is staffed by board-certified nurse practitioners and physician assistants that provide care for many treatments and services and write prescriptions as needed.
- We are accredited by The Joint Commission for the quality and safe care we provide to our patients.

- Our providers can send a summary of your visit to your primary care provider, with your permission.
How much does The Little Clinic cost?
Charges may vary based on the level of care needed to properly diagnose and treat a patient. Most of the time, visits fall within the range provided, but in more complex cases additional testing or services may be needed to properly diagnose a patient. If any labs are performed and sent to a third party laboratory the patient will be billed directly from the laboratory.
What should I bring when I visit The Little Clinic?
- Copy of your insurance card
- Proof of ID
- Valid form of payment at time of service
What ages can The Little Clinic treat?
The Little Clinic can treat patients aged 12 months old and up. Legal guardian consent is required for patients under 18 years of age.
How do I make an appointment at The Little Clinic?
Appointments can be made online or by calling 888–852–2567. Walk-ins are also accepted (as time allows)!
Can I access my records at The Little Clinic online?

What type of healthcare providers work at The Little Clinic?
The Little Clinic is staffed by nurse practitioners and physician assistants who are board-certified and fully licensed. Our healthcare providers are fully qualified to diagnose and treat common illnesses and minor injuries, prescribe medications as needed, conduct tests and interpret laboratory results, provide wellness and preventive care, perform physical examinations and health screenings, and administer vaccinations and injections. Our providers have direct access to collaborating physician relationships that support the delivery of quality healthcare. Our healthcare providers treat common illnesses and minor injuries. They also provide wellness and preventive care; risk assessments, screenings and vaccinations. Plus, they can prescribe medication when you need it.
Can The Little Clinic provider prescribe medications?
Yes, but only if the condition/diagnosis requires a prescription for treatment based on the provider’s assessment. Narcotic and other controlled substances are not prescribed at The Little Clinic.
